Introduction to the Learning Disability Teams

There are 2 Learning Disability Teams in Wakefield:

  • Learning Disability Team West;
  • Learning Disability Team East.

The teams support adults with learning difficulties and disabilities.

Children’s Services or health/social care professionals can refer direct to the teams when the person has a diagnosed learning disability. All other referrals are made through Social Care Direct.

The Learning Disability Teams procedures provide guidance to the locality teams around receiving a contact or referral, having a skilled conversation to establish needs, Care and Support Planning, arranging services and review.  Additional supplementary guidance around local processes and requirements can be found in the Local Resources.

Aside from the social work teams, there are three other learning disability services in Wakefield:

Service Role
Shared Lives Service A care and support service which is for people aged 17+ who want to live or have a short break with a shared lives carer.
Supported Living Services Supported living enables people with a disability to live independently in their own home with support provided.
Day Opportunities A service that aims to meet the needs and aspirations of people with severe learning disabilities from the age of 18, helping the person experience independent travel, education, work and/or supported employment.
