Assessment and Care Management consists of the following teams:

Role

Social Care Direct

The first point of contact for enquiries for all adults, triaged then to the appropriate service, works 24/7.

Adult Safeguarding Team

Specific team responding to any safeguarding concerns or provider concerns.

Hospital Social Work Team

Social Work team supporting in safe and timely discharges.

Locality East

Social Work Team providing support to older adults and adults with a physical disability across the East of the district.

Locality West

Social Work Team providing support to older adults and adults with a physical disability across the West of the district.

Continuing Health Care

A team that supports people with long term complex health and social care needs.

The Locality Teams and the Adult Safeguarding Team are accessed through Social Care Direct.

For other teams, referrals and interventions are made/agreed through liaison with other health and social care services.

The procedures in this section of the website should be used by all Care Management teams, except for Social Care Direct.

Practitioners working in Social Care Direct should see the dedicated section of the contents list for that team.

The Care Management procedures provide guidance around receiving a contact or referral, having a skilled conversation to establish needs, Care and Support Planning, arranging services and review. It also includes a link to the Safeguarding Adults procedure and the NHS Continuing Healthcare Procedure.
