Mental Health Social Work in Wakefield

Some parts of Mental Health Social Work in Wakefield is provided as part of an integrated service with the South West Yorkshire Partnership NHS Foundation Trust (SWYFT), where some other parts of the service are jointly commissioned with SWYFT.

1. Community Mental Health Team (CMHT) Older People (OP) East; - (Jointly commissioned);
2. Community Mental Health Team (CMHT) Older People (OP) West; - (Jointly commissioned);
3. Enhanced West (integrated);
4. Enhanced East (integrated);
5. Core West (integrated);
6. Core East (integrated);
7. Single Point of Access (SPA) (integrated).

There is also a regional service that provides Social Work Support - Forensic Mental Health Team.

All referrals must be made via the Single Point of Access (SPA). Referrals can be made by anyone.

• Flanshaw lodge EPH: A 26 bedded CQC registered older persons home offering accommodation and personal care to adults with dementia;
• Hazelgarth EPH: CQC registered elderly persons home offering accommodation and personal care to adults with dementia.
