Introduction to Provision Services

Adult Social Care Provisions Services are accessed through Social Care Direct. Their role is to ensure that people access the right equipment, adaptations and support to maintain independence at home. 

The following teams make up the Adult Social Care Provisions Service in Wakefield:

Role

Wakefield Equipment Service

Provides eligible people with the right equipment to support independent living, or support when discharged from Hospital.

Wakefield Wheelchair Service

Provides eligible people with Wheelchairs.

Sensory Impairment Team

Supports people who are deaf, deaf blind or hard of hearing.

Adaptations Team

Supports minor and major adaptations to peoples living environment and circumstances following an eligibility criteria

Assistive Technology

Provides technology to support someone’s independent living

Community Provision Service

A service that provides a service to adults who are struggling to do day to day tasks or who have long term illnesses

The procedures in this section of the website will provide guidance to all provision services around receiving a contact or referral, having a skilled conversation to establish needs, arranging equipment, minor and major adaptations, reviewing interventions and assistive technology and telecare.
